Full-time position Evening (3:30 PM - 12 MN) 5 days per week 8 hours per shift

Licensure : Active New York State RN license. Experience : Minimum 1 year of long-term care experience. Technical Skills : Proficiency in patient care documentation and safe medication administration. Soft Skills : Strong communication, empathy, and adaptability in fast-paced environments. Additional Requirements : Immunity to Hepatitis B (proof required). Candidates must have proven experience and working knowledge in the following areas:
EMR proficiency Conducting thorough clinical assessments Recognizing and responding to acute changes in condition and completion of SBAR Developing and updating baseline and comprehensive care plans Performing resident admission assessments and documentation Handling of controlled substances and documentation Coordination of care and supervising LPNs and CNAs Fall prevention, Elopement prevention, and response Conducting regular nursing safety rounds Nursing clinical documentation, task management, and follow-up Elder abuse prevention and reporting GG completion PRI and MDS Strong understanding of current best practices in long-term care settings
Key Responsibilities : Conduct patient assessments and implement individualized care plans. Administer medications and treatments per physician's orders. Monitor and document patient progress in EHR systems. Collaborate with healthcare teams to optimize resident outcomes. Ensure compliance with facility policies and regulatory standards.
