Position Summary:

Key Responsibilities:
- Provide occupational therapy services in a skilled nursing facility setting - Conduct patient assessments and develop individualized treatment plans - Implement therapeutic interventions to improve patients' functional abilities - Document patient progress and treatment outcomes in medical records - Collaborate with interdisciplinary team members including physicians, nurses, and other therapists - Educate patients and families on therapeutic techniques and home safety - Maintain current state occupational therapy licensure - Adhere to facility policies and procedures - Participate in care plan meetings and discharge planning - Ensure compliance with regulatory standards and documentation requirements - Work scheduled shifts from 08:00 to 16:30 - Provide quality patient care in accordance with professional standards
Job Requirements:
* Active, unencumbered license in CA
* Minimum of 1-7 years of relevant experience, depending on specialty
* Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
* Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
* Current Resume listing detailed work history (MM/YYYY format)
* Two current clinical references
* State-required professional license
* Physicial exam (completed within the past 12 months)
* TB skin test or Quantiferon Gold (within the past 12 months)
* Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
* Current Tdap and Flu vaccinations
* Must pass background check and drug screening
