Job Summary : Under the supervision of the Transportation Coordinator, is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.
Duties/Responsibilities
Essential Job Functions:
•Effectively communicates with participants, families, staff and other agencies and departments.
•Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures that wheelchair lifts and tie down equipment are properly operated and that participants are delivered to appointments in a timely manner.
•Follows established schedules, adjusts schedules to meet unanticipated changes, determines the safest and most efficient pick-up and delivery routes.
•Assists passengers on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise.
•Inspects van and maintains it in a clean, orderly and safe condition. Reports the need for maintenance.
•Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any change in participant condition to team members.
•Completes documentation as required
Physical Requirements : Prolonged periods of sitting at a desk and working on a computer. Sitting, Standing, Walking, Talking, sitting at a desk, and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands: Sedentary physical activity performing non-strenuous daily activities of an administrative nature.
A. Stand less than 33% of time on duty.
B. Walk less than 33% of the time on duty.
C. Sit between 33% and 66% of time on duty.
D. Use of hands to finger, handle or feel over 66% of time on duty.
E. Reach with hands or arms less than 33% of time.
F. Full ability to talk and hear.
G. Correctable vision required.
H. Able to lift/carry up to 15 lbs.
Minimum Educational and General Qualifications:
High School Diploma or GED plus one year experience transporting individuals with special mobility needs. Requires valid Chauffeur/CDL driver's license with good driving record. Must be able to work effectively in a team environment and operate handicap accessible vehicles.
