New Opportunity: Care Manager (RN/LPN) Hospital to Home Grant Program
Pediatric Home Service is seeking a compassionate and dedicated Care Manager (RN/LPN) to join our team in Roseville, MN. This exciting new role supports our Hospital to Home grant program, helping medically complex children transition safely from the hospital back to their homes while ensuring continuity of care, positive outcomes, and optimal healthcare utilization.
* Position: Care Manager * Location: Roseville, MN * Hours: Full Time (8a-5p) with some flexibility in scheduling. * Requirements: RN or LPN license in good standing, BLS/CPR certification, 23 years of nursing experience (pediatric experience preferred), and a background in social work is a plus.
JOB DETAILS
* Collaborate with families, medical teams, ancillary providers, and community resources to facilitate effective communication and optimal care for pediatric patients.
* Steward healthcare resources and family benefit plans to ensure financial and care efficiency.
* Assess and evaluate the service and resource needs of assigned patients and their families.
* Research and recommend services, therapies, or equipment to improve patient outcomes.
* Coordinate patient care by working with staff to plan and attend care conferences, ensuring that the patients needs are met in accordance with physician orders and PHS policies.
* Compile patient information for case management and outcome reporting, including results of clinical interventions.
* Serve as a resource to patients, families, and clinical staff to address clinical, community, and psychosocial needs.
* Review and write policies and procedures related to patient care and education.
* Ensure satisfaction and compliance with patient care by working closely with referral sources.
* Communicate additional resources (e.g., transportation) to patients and caregivers and coordinate services as necessary.
Administrative Responsibilities:
* Participate in departmental and corporate quality improvement activities to improve overall care quality.
* Attend staff meetings and contribute to discussions regarding improvements and claims data reviews with PHS leadership.
QUALIFICATIONS:
* Education & Experience: * Bachelors degree (B.A.) from a four-year college or university, or 1-2 years related experience and/or training, or an equivalent combination of education and experience.
* Licenses/Certifications: * CPR Certification
* Registered Nurse or Licensed Practical Nurse licensed by the MN State Board of Nursing (or equivalent clinical licensure)
* Preferred Qualifications: * Background in social work or a social work degree.
* Experience in EHR, homecare, home health, infusion, complex rehab, or pediatric advocacy groups.
