Care Manager RN - Case Management at Providence Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8-hour Day shifts.

Providence Petaluma Valley Hospital provides outstanding patient care which has garnered our placement on the Maternity Honor Roll for the California Health and Human Services Agency. Join our reputable team and be part of a healthcare institution known for its clinical excellence and compassionate care.

Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.

Required Qualifications

Upon hire: California Registered Nurse License 2 years Acute care clinical or comparative experience

Preferred Qualifications

Upon hire: CCM 3 years experience in community health nursing or case management
