JOB SUMMARY

The RN Navigator of the NCH Palliative Care Program works with the NCH Palliative Care Team independently to optimize patient care and experience, as well as provide an extra layer of support to patients with serious illnesses and their families. The RN Palliative Care Navigator provides professional care to those seriously ill patients who have been referred to the NCH Palliative Care Team and is responsible for coordinating optimal patient and family experiences from time of referral through discharge and follow-up after discharge by phone and/or in the NCH Outpatient Patty Baker Palliative Care Clinic. A database will also be established and managed to track all relevant patient information and activity.

ESSENTIAL DUTIES AND RESPONSIBILITIES

NCH Inpatient Palliative Care Program • Works with the NCH Palliative Care Team, and independently, to assess patients' symptoms and understanding of their condition, as well as patients' and families' social and emotional needs. • Facilitates a timely discharge in coordination with the case manager/social worker. • Communicate regularly with the Palliative Care Team, nurses, referring physicians, etc. • Assist patients in completing the advanced care directive documents • Assist with discharge planning including prescription management, follow-up appointments including in the NCH Palliative Care Clinic if indicated. Explore if prior authorization for pain medication is needed • Follow-up with patients and families after discharge • Provide education to nursing staff and medical technicians on palliative care topics to improve their understanding of palliative care and symptom management, including pain.

NCH Outpatient Palliative Care Program • Return phone calls regarding patient concerns including medication management and increased symptoms • Be available to assist outpatients with special needs such as transition of care to hospice or hospital.

NCH Palliative Care Academic Program • Participate in weekly team/fellow education • Participate in monthly journal club

NCH Palliative Care Evolving Home Palliative Care Heart Failure Program • Meet with patients in the hospital who have been referred to the NCH Home Palliative Heart Failure Program • Follow-up with home patients on a regular basis to optimize their symptom management, understanding of their condition and assist with transitions of care when necessary.

Additional Responsibilities: • Practice ethically • Practice in an environmentally safe and healthy manner. • Identify opportunities for practice improvement • Active review and facilitation change of management opportunities identified during data presentations • Develop and deliver reports related to trends, outcomes and objectives. • Maintain currency with all required compliance training programs. • Maintain a commitment to professional growth and competency including pursuit of related certifications. • Actively participate in research and quality improvement projects that arise • Maintain open communication with the leadership at NCH Healthcare Systems, including NCH Medical Group regarding quality improvement, protocols and finances (quarterly Quality and Finance meeting). • Develops and implements educational materials for patients and families to ensure consistency in information presented, taking into consideration their learning needs and barriers to learning, and documents the education provided.

EDUCATION, EXPERIENCE AND QUALIFICATIONS • BSN Preferred • Minimum of 2 years of experience in hospital experience required. • Licensed as a Registered Nurse (RN) in the State of Florida • Basic Life Support (BLS Provider) certification required from American Heart Association • Communication and Problem-Solving skills and ability to maintain flexibility • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
