Full-Time Outpatient Nurse Coordinator I
University of Pittsburgh Physicians Department of Neurological Surgery is hiring a full-time Outpatient Nurse Coordinator I to join their team at Presbyterian Hospital (200 Lothrop Street, Suite B400, Pittsburgh, PA 15213). In this position, you will work Monday-Friday from 8:00am-4:30pm. This role provides opportunity to work from home 1 day per week after successfully completing an orientation period/6 month of employment. We're seeking a dedicated and compassionate Registered Nurse to join our outpatient care team. Purpose: The Outpatient Nurse Coordinator plays a vital role in delivering high-quality, holistic care to patients across the continuum of outpatient services. This position requires independent clinical judgment, strong communication skills, and a commitment to patient advocacy. You'll collaborate closely with physicians, families, and community resources to ensure patients receive comprehensive support both during and between visits. Key Responsibilities: Patient-Centered Care:
Develop individualized care plans in partnership with patients and families. Triage patient needs and coordinate care with physicians and external services. Serve as the primary nursing resource for a designated patient caseload. Provide education and guidance to patients and families throughout their care journey.
Clinical Leadership:
Supervise and support nursing staff, medical assistants, and office personnel. Lead daily clinical and administrative operations within the practice. Mentor new staff and contribute to their professional development.
Professional Practice:
Engage in continuous learning and shared governance initiatives. Participate in quality improvement efforts and regulatory preparation. Apply evidence-based practices to enhance patient outcomes and satisfaction.
Collaboration & Communication:
Foster positive relationships with interdisciplinary teams and external partners. Advocate for patients and communicate effectively across all levels of care. Maintain thorough and accurate documentation to support care coordination.
Education & Teaching:
Provide patient and family education using adult learning principles. Support training and educational activities for staff and students. Serve as a preceptor and role model for clinical excellence.
Minimum 3 years nursing experience BSN preferred Licensure, Certifications, and Clearances: Current licensure as a Registered Professional Nurse in practicing state. CPR Certification UPMC approved national certification preferred.
