Position Summary:Under the direction of the Center Manager and day-to-day supervision of the Day Center Manager, the Activities Coordinator will be responsible for oversight of the Day Center Activities and ensuring the Activities Team carries out the activity program. Responsibilities include coordinating, supervising, and implementing all day center activities and social programs for frail elderly and functionally impaired older adults at the Day Center in an integrated program setting. All policies and procedures will be followed at all times. The Activities Coordinator will be responsible for the development and implementation of daily, monthly and special events activities, outings, birthday celebrations, etc.Essential Functions of the Job:Plans a comprehensive program tailored to the needs, interests and limitations of frail elderly persons, to promote optimum health, independence and self-care; increase or maintain social skills and self-esteem.Evaluate all participants' functional status in relationship to participation in recreational activities. Establish realistic goals and a treatment plan to utilize recreation therapy techniques. Re-evaluate patients regularly.Collaborate with other staff members (social workers, physical therapists, occupational therapists, CNAs, etc.) to develop care plans that meet participant's needs, resources, and interests.Collaborate with the interdisciplinary team to determine the Plan of Care for participant/member.Complete required progress notes and six-month reassessments for participants.Provide recreational therapy opportunities; instruct family and others on therapeutic activities as indicated.Provide general instruction on the principles of recreation therapy and therapeutic activities as indicated.Supervise and coordinate Program Aids, Activity Aides, Community Service providers, and volunteers; make evaluation recommendations to the Program Director.Develop and post a monthly calendar of events.Assist with in-service trainings and planning of community events.Develop and work within an Activity Program budget approved by the Program Director; submit purchasing requests in advance.Establish an activity volunteer program and coordinate with community organizations to support the Activity Program.Assist participants with personal care as needed.Stay current with regulations and standards to ensure compliance with CMS, DHCS, etc.Meet administrative requirements including data capture and report submission.Conduct performance evaluations, schedule staff/volunteers.Be proficient in EHR; responsible for daily documentation and care plan development.Conduct home, hospital, and SNF visits/assessments as needed.Travel to other sites as requested.Participate in scheduled outings.Additional Duties and ResponsibilitiesEnhance professional growth through education, literature, workshops, etc.Model and adhere to core values and behaviors of Excellence, Empowerment, Integrity, and Respect.Adhere to attendance and punctuality policies.Follow all organizational and departmental policies and procedures.Attend meetings and serve on committees as directed.Maintain safety standards and participate in safety programs.Perform other related duties as assigned.Job RequirementsMinimum Qualifications:One year experience in a social or recreational program in the past five years, preferably in senior care.Must be a certified activity coordinator or equivalent professional; or have completed 36 hours of specific training with regular consultation.Flexible, creative, organized, with strong time management.Bilingual (English/Spanish) preferred.Universal RequirementsPre-employment requirements include I-9, physical, background check, references, application, orientation, and health screening.HIPAA/ComplianceMaintain privacy of all information; access on a need-to-know basis.Report unethical, fraudulent, or unlawful behavior.OSHA ComplianceFollow OSHA training and schedules.PPD ComplianceAnnual health screening required.Policies and ProceduresFollow all organizational policies and procedures.Meal Period Compliance (Non-exempt only)Clock out before 5 hours from start of shift.Sexual Harassment Training (Management only)Complete required training per schedule.
