Certified Peer Outreach Specialist Case Manager
Mercy Care Atlanta is seeking a compassionate and dedicated Certified Peer Outreach Specialist Case Manager to join our outreach team. This role is designed for individuals with lived experience in mental health and/or substance use recovery who are passionate about supporting persons experiencing homelessness (PEH). Through street outreach and peer-based engagement, you will help individuals navigate recovery, access essential services, and build self-advocacy skills. What You Will Do: Conduct outreach in encampments, under bridges, and on sidewalks to engage individuals facing mental health and/or substance use challenges. Use your lived experience to provide emotional support, motivation, and recovery coaching. Assist clients in navigating healthcare, behavioral health, and social service systems. Facilitate connections to treatment programs, recovery services, and peer support groups. Maintain accurate documentation of interactions and progress while upholding confidentiality. Collaborate with outreach teams and service providers to coordinate care and advocate for client needs. Manage an active caseload of approximately 2025 clients, providing consistent follow-up and support. Minimum Qualifications: High School Diploma or equivalent. Certified Peer Specialist (CPS) certification from the State of Georgia. At least 3 years of experience in mental health or social services. Strong communication, problem-solving, and interpersonal skills. Compassionate, non-judgmental, and culturally sensitive approach. Valid Georgia driver's license; ability to operate company vehicles, including a 12-passenger van.
