Overview
The Care Quality Navigator works indirectly with primary care office staff and providers to provide administrative support and lessen the burden of payor quality programs on physician offices. Primary duties include assisting with payor data entry to close care gaps, outreach to patients on behalf of primary care offices, and assisting with office meeting preparations.

Responsibilities
Navigating healthcare and patient education
- Providing patients with information about their health conditions, treatment options, and self-management strategies.
- Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements.
- Provide patient outreach support (via phone) and assist primary care offices with any additional patient outreach needed. Track patient attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.
- Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance.
Care coordination
- Provide office support in closing care gaps.
- Communicating with various healthcare providers to ensure seamless care delivery and avoid unnecessary duplication of services.
- Communicate with team members, physicians, advanced practice practitioners and other health care providers to facilitate patient care.
- Serves as contact for primary care physicians to refer patients for care coordination and care management services and communicates essential instructions given by provider.
Quality data analysis
- Performs a wide range of clerical duties while assisting provider offices in giving quality care in a medical office setting.
- Performs payor data entry for the primary care offices according to Covenant HealthCare policies.
- Accurately documents all patient care interactions and track patient information in a confidential manner.
- Attend and represent the organization at professional conferences, in-service trainings, and meetings.
- Performs all other duties as assigned.
Qualifications
EDUCATION/EXPERIENCE
High School diploma and/or equivalent required,
AND
a successful completion of a competency-based Medical Assistant (MA) training program required.
OR
one (1) year experience in clinical (direct patient care) and/or medical (administrative patient support) required.
Physician office and EMR experience (EPIC) preferred.
KNOWLEDGE/SKILLS/ABILITIES
-Excellent communication and interpersonal skills
-Ability to maintain confidential information
-Excellent organization, time management skills and attention to detail.
-Ability to remain calm and courteous in stressful/difficult situations.
-Proficiency with basic computer and typing skills.
-Ability to work independently or as part of a team.
WORKING CONDITIONS/PHYSICAL DEMANDS
-Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
-Constant sitting, handling, feeling, talking, hearing, tasting, smelling, and near vision.
-Frequent lifting up to 25 lbs.
-Frequent depth perception, and field of vision.
-Occasional lifting up to 50 lbs. or more.
-Occasional standing, walking, lifting, carrying, pushing, pulling, stooping, kneeling, crouching, squatting, crawling, and far vision.
