Home Based Primary Care Social Worker
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Total Rewards of a Home Based Primary Care (HBPC) is a home program that provides comprehensive, interdisciplinary primary care in the homes of veterans with complex medical, social, and behavioral conditions from whom routine clinic-based care is not effective. HBPC is designed to serve the chronically ill through the months and years before death, providing primary care, palliative care, rehabilitation, disease management, and coordination of services. The HBPC Social Worker is licensed at the advanced practice level with at least two years in an area of specialized social work practice and is able to provide supervision to staff to qualify for licensure, when needed. The HBPC Social Worker must be licensed or certified by a state at the advanced practice level which includes Association of Social Work Boards (ASWB) advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level. The HBPC Social Worker provides direct services to adult and geriatric veterans enrolled in HBPC Program with high quality case management, clinical treatment, advocacy, and coordinates linkages with appropriate VA and community service providers/agencies as needed by the client working independently in the community with minimal supervision. This is accomplished in collaboration with other members of the HBPC Team or other interdisciplinary treatment teams as appropriate (i.e. Specialty Clinics, Community Living Center Units, Primary Clinics, etc.) where the social worker provides subject matter consultation to colleagues and/or students. The social worker possesses advanced knowledge of theories and modalities used in the specialized treatment of complex physical or mental illness and can make psychosocial and psychiatric diagnoses within approved scope of practice or clinical privileges. The social worker possesses advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans and their family members. The social worker is responsible for assessment, planning, and provision of case management services in the HBPC Program. The Clinical Social Worker coordinates a variety of activities including outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Medical Center staff and community based service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervision of students as assigned. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. A valid driver's license is required. For official travel, the incumbent must be open to operating a government vehicle when available. Qualifications: BASIC REQUIREMENTS. To qualify for appointment as a social worker in VHA, all applicants must meet the following: a. Citizenship. Be a citizen of the United States. b. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. c. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. d. Grandfathering Provision. All persons employed in VHA in this occupation on the effective date of this qualification standard (9/10/2019) are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. e.
