The Hospital Liaison plays a crucial role in ensuring a seamless transition for all admissions from acute or post-acute care. Your collaborative engagement is vital to coordinate admissions and communications to maintain continuity and ensure the best care our communities expect. You will generate and manage referrals and admissions from hospitals, develop relationships with hospital discharge planners, and initiate strategies to grow business and identify new opportunities. Hospital Liaisons work closely with our AD Concierge and leadership to drive daily admissions. Strong time management and multitasking skills are essential for market growth. Our staff and residents will rely on your knowledge and attention to detail to achieve the best patient experience. Role Responsibilities: Stationed daily at primary accounts to maximize market engagement and referral review efforts. Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms. Respond promptly to all incoming referrals on Careport and other platforms, daily and throughout the day. Provide definitive answers for all incoming referrals within one hour of receipt or by end of day if appropriate. Collaborate with Business Development and Operations leadership to resolve delays and barriers in real time. Coordinate with AD Concierge on daily updates regarding pending referrals for Forecast. Work with Transitional Advisors on communication with incoming referrals and admissions on CarePort and other platforms. Input patient data timely, including demographics, resident representatives, POA, and guardian information. Prepare weekly reports. Participate in quarterly Business Development meetings to align initiatives that grow census and market exposure. Complete daily Forecast updates for team review via Census calls, morning meetings, or standdowns. Oversee Guest Relations and Concierge initiatives, including patient rounding, testimonials, and promoting positive outcomes. Qualifications: Minimum of a GED or High School Diploma with 2 years of experience; an associate degree in healthcare, business, or a related field with 1 year of experience is preferred. Must meet health assessment requirements regulated by the state. Ability to perform essential job functions.
