Job Summary We are seeking a
Direct Care PRP Counselor
to join our team and support youth and adults with mental illness in enhancing their quality of life. As a PRP Counselor, you will provide essential services to help clients achieve their rehabilitation goals, including assistance with daily living skills, personal hygiene, organization, building supportive networks, and promoting a positive and supportive living environment. The ideal candidate is patient, compassionate, reliable, and committed to fostering independence and empowerment for individuals in need.
Responsibilities
Meet with clients for individual and group sessions.
Participate in all treatment planning, communicate consistently with clinical staff, Program Director, and clients' families.
Attend and complete Ascend's required training.
Maintain contact with the client’s family and treatment team.
Implement rehabilitation activities.
Assist clients in achieving their individual goals by teaching and supporting them while linking them to various community resources.
Maintain quality records and timely documentation.
Qualifications
At least 2 years of experience working with mentally ill children or adults.
Complete & pass a federal CJIS background check.
Valid driver’s license and reliable transportation.
Ability to adhere to all health and safety guidelines.
Excellent communication and interpersonal skills.
Ability to maintain quality records and timely documentation.
Compassionate, respectful, and ethical attitude.
