Summary
Job Summary
Responsible for the execution of the clinical aspects of the Case Management process.
Provides outreach and enrollment services to meet eligibility requirements of program and surrounding counties.
Ensures coordination and planning with the help of community and family support. Focuses on the development and coordination of community service plans.
Partners with service providers, families, and patient to create plan of healthcare and identifies additional service providers.
Responsible for overseeing customer service, supervisor and role model for staff, the daily assignment of staff, patient flow and shift to shift decision making regarding staffing.
Participates with leadership in the development of the case management standards, evaluation of outcomes and development of improvement initiatives.

Essential Functions
Provide day-to-day supervision to Case Managers and interns/students as appropriate.
Complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status. Review daily referrals and screenings of clients. Audit case file on a regular basis for completeness and accuracy. Assist in discharge planning as required. Serves and protects the hospital community by adhering to professional standards, hospital care policies and procedures, federal, state, and local requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.

Education
Bachelor's Degree Nursing or Master's Degree Social Work or at least Master's Degree in a Therapy Related Field of Study required

Licensed Independent Clinical Social Worker [LICSW - State License] - Generic - HR Only preferred Registered Nurse [RN - State License] - Generic - HR Only preferred
Experience
Supervisory or management experience with professional staff in a healthcare setting 5-7 years required Experience in case management, utilization review, or discharge planning 5-7 years required
