Purpose: The Transition Therapist is a member of the interdisciplinary team providing basic clinical services to persons with mental illness or physical disabilities targeted to transition from long term care to the community. The therapist will provide counseling and supportive services to assist class members that experience fear or anxiety around their move.

Essential Responsibilities, include but are not limited to: Conduct assessments Provide on-site, supportive counseling to CM’s residing in nursing homes and SMRFS Utilize motivational interviewing and other techniques to assist CM’s make decisions and long-term goals Assist CMs identify crisis triggers, and contingency plans in the event a crisis arises post discharge from the facility. Maintain and update client’s pertinent information within the MH electronic care system i.e. face sheets, contacts, consents, and diagnoses. Achieve required productivity targets and complete documentation in a timely manner, including clinical documentation and service reporting. In conjunction with the client’s treatment team, keep client’s family, guardian, and other Envision Unlimited staff informed of the client’s progress, including any critical incidents, emergency room visits, hospitalizations, and medication

changes/adjustments

you become aware of. Provide information to outside agencies as requested to coordinate and assure the appropriateness of services. Provide and document crisis interventions as needed, within 24 hours of an incident, and complete an incident report when necessary. Requirements:

MINIMUM QUALIFICATIONS:

Master’s degree in social work, counseling, psychology or related field with at least three years’ experience in social services. Must have excellent written and verbal communication skills, sufficient decision making, problem solving, computer and organizational skills. Individual must be able to work autonomously. Must have a valid driver's license, and ability to pass background check.

Compensation & Benefits: Base Salary:

$65,000 per year Paid Time Off:
