Patient Care Coordinator . This is a
contract-to-hire

Job Description: The Patient Care Coordinator will be responsible for managing the end-to-end coordination of patient medication deliveries, processing supply orders, verifying insurance benefits, and supporting pharmacy operations. The role involves close collaboration with pharmacists, intake staff, and provider offices to ensure timely and accurate service to patients.
Key Responsibilities: Schedule deliveries with patients or caregivers and confirm product needs Process and package prescriptions and supplies for delivery Support pharmacists with medication prep and dispensing Enter and update orders in internal systems across multiple platforms Confirm delivery completion and maintain accurate documentation Communicate with intake teams and physician offices to complete orders Verify insurance benefits and handle prior authorizations Direct patients to clinical staff when necessary for follow-up care Ensure compliance with policies and maintain excellent patient service Qualifications: Active
Pharmacy Technician License
(state-specific) and
National Certification (CPhT) Minimum
2+ years’ experience in pharmacy operations or home infusion setting Strong knowledge of
PBM processes and pharmacy-specific terminology Familiarity with tools like
Availity ,
CoverMyMeds , and other insurance portals Comfortable navigating commercial, Medicaid, and limited Medicare payors Excellent attention to detail, multitasking ability, and communication skills Team-oriented mindset and passion for patient care Additional Details: Type:
Contract-to-Hire (600 hours) Schedule:
Monday to Friday, 8:00 AM – 5:00 PM CST Work Setting:
Hybrid – 2 days per week onsite Start Date:
ASAP Parking:
