Responsibilities Oversee daily operations and supervise provider staff. Ensure availability of Clinical and/or Executive Director during all operating hours or provide coverage as needed. Be immediately available on-site or by telephone during off-site activities, including community presentations, conferences, and physician meetings. Available after hours as required. Supervise and ensure high-quality patient care throughout the episode of care, including referral coordination, ongoing patient needs assessment, and development and updates of individualized care plans. Coordinate with physicians, families, and other clinicians involved in care. Communicate effectively with staff through meetings, case conferences, and in-services. Supervise patient care to ensure safety, efficiency, and compliance with relevant standards, regulations, and policies, including adherence to the plan of care ordered by physicians. Education and Experience Registered nurse with at least 1 year of supervisory or administrative experience in home healthcare or related fields. Current CPR certification required. Valid driver's license, vehicle insurance, and access to a reliable vehicle. For AZ: At least 3 years of nursing experience, including 24 months as an RN in home health services.
