About Your Role:
As a Home Health Physical Therapist Care Manager, you will manage a caseload of patients in the Downtown Orlando, Pine Hills, West Orlando, Ocoee, Winter Garden, and Windermere areas (including zip codes: 34760, 34787, 34734, 34786, 34761, 32818, 32835, 32819, 32811, 32808, 32810, 32714, 32804, 32805, 32839, 32806, 32801). You will conduct home visits to assess and develop personalized care plans tailored to each patient's needs.
Your Responsibilities Include:
- Coordinating and directing care for home health patients requiring physical therapy.
- Conducting comprehensive assessments and developing individualized, realistic plans of care in collaboration with patients, families, and physicians.
- Performing regular evaluations and documenting patient responses to treatment, ensuring adherence to the care plan.
- Adjusting treatment plans as necessary based on ongoing assessment and patient progress.
- Participating in interdisciplinary conferences and making appropriate referrals to enhance patient outcomes.
Qualifications Needed:
- Master's Degree in Physical Therapy.
- At least 1 year of relevant clinical experience in physical therapy.
Preferred Qualifications:
- Doctorate Degree in Physical Therapy.
- Recent experience in a Medicare-certified home health agency.
- Certification for Oasis Specialist-Clinical (COSC).
- Home Health Care Manager Certification.
