What You'll Do
Conduct
in-home supervisory visits
to monitor care, safety, and performance Provide
real-time coaching and mentoring
in ADLs/IADLs, dementia care, fall prevention, and infection control Support caregivers with retraining, recognition, and advocacy Document supervisory visits in our
CRM/EVV system
same day Ensure compliance with EVV, HIPAA, and professional standards Collaborate with the
Nurse Director and Scheduling Team
for caregiver-client matches Escalate safety concerns and participate in
Root Cause Analysis (RCA)
follow-ups Rotate flexible schedules (day/evening visits, every other weekend) What We're Looking For
CNA, LPN, or RN
with supervisory or leadership experience 5+ years caregiving or clinical experience
(home care, assisted living, rehab, or long-term care) Strong knowledge of dementia care and chronic condition support Excellent communication and leadership skills Tech-savvy with mobile documentation (CRM/EVV) Valid driver's license + reliable transportation (company vehicle may be available) CPR preferred
