We are seeking a
Care Navigator . The position assists participants with the following services (but not limited to): assess need; linkage to resources and treatment providers; coordinates care among community services and systems of care; removes barriers for successful engagement; assistance with benefits; secure housing within the community and assists in monitoring their placement to insure stability of housing.
Education/Experience/Other Requirements: High school diploma required. Four-year degree in social work, psychology, or related field preferred. Prior experience preferred in mental health, community resources, and the justice system. Minimum of a CDCA license required. LCDC II/III or LICDC preferred. Must be able to work a flexible schedule that may occasionally include evenings and weekends. Primary Duties:
The Navigator will be placed within the Cherry Street Ministry Mission. The position works to assist clients (assigned caseload) to secure housing within the community and assists in monitoring their placement to ensure stability of housing. Completes appropriate documents to ensure eligibility for and enrollment in program. Performs case management duties, assisting clients with overcoming barriers of obtaining and maintaining resources. Maintains confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures. Must demonstrate knowledge in chronic physical health issues and needs of persons who are SMI or SED. Must have knowledge of the justice system and the intersection of those with mental illness and SUD. Must demonstrate a positive attitude and support for the mission of the agency. Must possess personal integrity and the ability to work as part of a team. Must be able to work with ill, disabled, emotionally upset and at times emotionally dysregulated individuals. The ability to build relationships, collaborate and coordinate with external providers as needed. Participates in team meetings, supervision, in-service training, and conferences.
