Care Coordinator

to join our team and support both client care coordination and daily office operations.

Position Summary

This hybrid role is ideal for someone who is highly organized, detail-oriented, and people-focused. The

Care Coordinator

will oversee client scheduling, caregiver matching, care plan management, and in-home visits, while also ensuring smooth day-to-day office operations. The role requires excellent communication skills, problem-solving abilities, and a passion for helping others.

KEY RESPONSIBILITIES

Care Coordination:

Manage client scheduling and match caregivers based on skills, availability, and client needs.

Conduct follow-ups with clients and families, including

in-home visits , to ensure quality of care and satisfaction.

Maintain accurate client files, assessments, and care plans in compliance with company and state requirements.

Support recruitment, onboarding, and training of caregivers.

Provide caregivers with schedules, updates, and support for client-specific needs.

Support on-call coverage needs on a rotational basis.

Office Administration:

Oversee front office operations, including phone calls, emails, and visitor inquiries.

Handle data entry, filing, and maintenance of office records.

Assist with billing, payroll support, and insurance/Medicaid documentation as needed.

Prepare reports, manage supplies, and support compliance with policies and procedures.

Coordinate communication between caregivers, clients, and management.

Qualifications

Previous experience in

care coordination, client care management, or healthcare administration

preferred.

Strong organizational and multitasking abilities.

Proficient in Microsoft Office Suite; experience with

ClearCare/WellSky

or similar scheduling software is a plus.

Excellent verbal and written communication skills.

Compassionate, professional, and reliable with a strong attention to detail.

Ability to handle confidential information and work under pressure.

Valid driver’s license and reliable transportation for

in-home visits .
