About the Role: As a Physical Therapist, you will provide comprehensive, empathetic care to patients in Home Care and Hospice settings. You'll work closely with patients, families, and interdisciplinary care teams to improve mobility, reduce pain, and enhance quality of life—often in the comfort of the patient's home. This is a travel-based position within a designated territory. A company vehicle or mileage reimbursement is provided. Key Responsibilities: Evaluate patient mobility, muscle strength, and rehabilitation potential

Develop and implement customized therapy plans in collaboration with the care team

Educate caregivers and patients on exercises, equipment, and safety protocols

Document progress and revise therapy plans as needed

Contribute to discharge planning and provide consults as necessary

Supervise PT Assistants and provide in-service education to clinical staff

Qualifications: Graduate of an accredited Physical Therapy program (Bachelor's or Master's level)

Licensed Physical Therapist in the state of New Hampshire

2+ years of PT experience preferred; home care or hospice background a plus

Strong interpersonal and documentation skills

Valid driver's license, reliable vehicle, and active auto insurance

Current CPR certification

Working Conditions & Physical Requirements: Requires frequent travel within the region

Involves lifting, patient transfers, and physical activity

Must be comfortable with a variety of weather and home conditions
