Job Summary: The registry pediatric pharmacist provides clinical and operational pharmacy services on an as-needed basis to support pediatric patient care across inpatient and outpatient settings. This flexible, per diem role requires a pharmacist with pediatric experience and strong clinical judgment to contribute to safe and effective medication use. The pharmacist will support admission and discharge medication coordination, outpatient services, and clinical interventions during variable shifts including weekends, evenings, and holidays. Essential Registry Pediatric Pharmacist Job Functions: Review and verify medication orders for pediatric patients, ensuring safe, effective, and evidence-based use. Provide age- and weight-appropriate dosing recommendations and drug monitoring. Identify and resolve medication-related issues and provide drug information to healthcare professionals. Conduct medication reconciliation during patient admissions. Review and counsel on discharge medication plans to ensure clarity, safety, and accessibility. Educate patients, families, and caregivers on medication administration and potential side effects. Collaborate with ambulatory care providers to resolve drug access, adherence, and authorization issues. Assist with continuity of care across transitions between hospital and home or outpatient settings. Verify and oversee pediatric compounding, including sterile and non-sterile preparations. Assist with inventory control and ensure proper handling of pediatric-specific medications. Collaborate with technicians to ensure timely and accurate medication distribution. What we need from you: Education:
Pediatric experience in a hospital or clinical setting; completion of PGY1 Pharmacy Residency preferred Related Work Experience:
Pediatric inpatient and outpatient pharmacy experience Licensure/Certification:
Pharmacist credentialed from the Illinois Board of Pharmacy obtained prior to hire date or job transfer date required. PALS, ACLS What you will receive: Competitive Salary

Compensation:
Pharmacist I: less than 2 years of Hospital Pharmacy experience, Pay Rate - $70/hr Pharmacist II: 3-5 years of Hospital Pharmacy experience, Pay Rate- $80/hr Pharmacist III, greater than 6 years of Hospital Pharmacy experience, Pay Rate - $90/hr Who we are: La Rabida is a place unlike any other.
