About the Position
Based at our Roxbury site, the Family Support & Stabilization Clinician partners with the Massachusetts Department of Children and Families (DCF) to provide home- and community-based services that strengthen family functioning and safety. You will conduct weekly (1–2x) home visits, complete timely assessments and treatment plans, coordinate with DCF and community providers, and deliver concrete supports across parenting, medical/mental health,
education/vocation,
and legal/justice-related needs. This role requires strong documentation, flexibility for some after-hours needs, and collaborative advocacy for parents/guardians and their children.
Key Responsibilities
Complete immediate assessments; develop, implement, and update treatment plans aligned with DCF goals. • Conduct weekly home visits (1–2x/week) and provide ongoing case management, advocacy, and referrals. • Document all contacts and visits in the Efforts to Outcomes (ETO) system; produce monthly reports every 30 days. • Use the Virtual Gateway system to complete treatment plans each 3-month period and complete end-of-service progress reviews. • Maintain neat, orderly, and compliant case files; protect confidentiality and adhere to mandated reporter requirements. • Collaborate weekly with DCF and other collaterals; participate in DCF case conferences. • Respond to urgent needs (e.g., child crises, hospitalizations, emergency/school meetings) as necessary. • Demonstrate professional representation, appropriate boundaries, strong communication, and effective teamwork. • Participate in weekly supervision with the Program Director.
Requirements
Must-Haves • Bachelor’s degree in Social Work or a related Human Services field. • At least 2 years of experience providing case management and/or counseling to children, adolescents, and families (home- or community-based preferred). • Valid driver’s license, properly insured/reliable transportation, and willingness to travel for home visits across Greater Boston. • Willingness to be on call after hours for emergencies.
Nice-to-Haves • Master’s degree in Social Work or related Human Services field. • Professional licensure (e.g., LCSW, LMHC) or eligibility to obtain within 6–12 months. • Additional language skills beyond Spanish (e.g., Haitian Creole, Cape Verdean Creole, Portuguese).
