Position Summary Under limited supervision by the Chief of Adult Medicine, the Physician provides the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care and health maintenance to patients. The Physician serves as the leader within the patient centered medical home model and work collaboratively with the care team to ensure that each patient gets the best possible care.
Essential Duties and Responsibilities Leads the primary care team in providing patients with primary medical services within the standard of care for Internal Medicine, (Family Practice or Osteopathic Medicine.) Participates in daily care team huddles to ensure prioritization of patient needs. Attends to patients in a timely and orderly manner to keep waiting time to a minimum and maximize productivity. Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments. Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner. Ensures documentation is recorded in a complete, timely and orderly manner, utilizing template in the electronic medical record. Adheres to UDS productivity and quality standards and meets average minimum requirements of 3200 billable encounters annually. Performs comprehensive examinations, including physical, developmental assessment and screening for psychosocial problems. Complete timely documentation in the EHR for each patient encounter and review patient medical records for accuracy, and completeness as outlined in BJHCHS Policy for ensuring timely medical record documentation. Requests, obtains and evaluates results of screening tests. Prescribes treatment plan for management/care for patients and families according to standards of care guidelines and refers appropriately. Develops a health maintenance plan for individual patients which includes health education, health promotion/disease prevention and general anticipatory guidance. Counsels patients and families appropriately regarding management of their care. Monitors the treatment regimen in selected populations. Assessed by medical and clinical peers through (quarterly) record audits and one on one training and observation, at least once a year, or if needed. Confer with Chief of Adult Medicine when needed for consultation, information, etc. Provides ongoing staff education based on health needs of population served. Provides individualized clinical orientation for nursing personnel in specialty area. Conduct quality assurance review utilizing clinical measures by life cycle. Serves as supervising physician for Nurse Practitioner or Physician Assistant if needed, and signs BJH Non-Physician Collaborative Practice Agreement to establish medical aspects of care to be provided by NP/PA including prescribing medications. Serve as advocate for the center and serve as liaison to local hospitals, health organizations as appropriate. Performs other related duties as assigned by management
Qualifications Medical degree in Internal Medicine, Family Practice, or Osteopathic Medicine from accredited college or university. Completion of a residency program in Internal Medicine or Family Practice Board certification Possess a South Carolina Medical License Possess a State Controlled Substance and (a) Federal DEA Controlled Substance registration. Current CPR certificate required
