About the Role: As a Targeted Case Manager, youll support individuals and families experiencing homelessness by helping them navigate barriers to housing and stability. Youll work closely with guests and community partners to develop housing plans, provide referrals, and offer trauma-informed, strengths-based support. This is an opportunity to make a real impactone person, one family, and one child at a time. Schedule:Full-time, MondayFriday Pay:$20$24/hour Sign on bonus paid at $750.00 at 90 days and $750.00 at 365 days of service What Youll Do: Provide individualized case management, care plans, and referrals for guests in crisis Use a harm-reduction lens to support people facing mental health, substance use, and complex trauma Maintain accurate documentation in HMIS and other required systems Build relationships with landlords, healthcare providers, and other agencies to remove barriers to housing Support guests for up to 90 days post-shelter to promote housing retention Ensure new guests feel welcomed, safe, and respected What Were Looking For: Bachelors degree in a related field (or equivalent case management experience for Section 13 eligibility) Experience with mental illness, substance use, or homelessness Strong interpersonal, communication, and organizational skills A commitment to housing first, harm reduction, and the mission of ending homelessness What We Offer: Mission-driven, inclusive culture Generous PTO + 13 paid holidays Health, dental, and vision insurance Flexible scheduling & mileage reimbursement 403(b) retirement plan + short-term disability Employee Assistance Program Public Student Loan Forgiveness eligible Advancement opportunities We are committed to equity and inclusion and welcome applicants from all backgrounds and experiences.
