Collaborates with providers and multidisciplinary care team members to provide care coordination across the continuum. Assesses and supports physical symptoms. Provides psychological support to patients, friends, and families. Mentors assigned team members. Promotes best practices and maintains professional disposition at all times. Provides quality competent care, following best practice standards. Works to achieve care goals and quality outcomes while following the standards of care set by the state board of nursing. Provides effective leadership in the delivery of quality nursing care by assessing, planning, organizing, and directing nursing functions. Collaborates with patients to implement their plan of care. Maintains the confidentiality of medical records. Ensures documentation standards are met with accurate, timely, and complete recording and reporting. Acts autonomously as a vital component of the care team. Delegates appropriate responsibility to the skill level of the employee. Identifies patient care needs to accomplish a high standard of care in collaboration with the interdisciplinary team. Functions as an advocate for those served. Creates a compassionate, service-oriented environment. Provides emotional, psychological, and spiritual support to patients and caregivers. Shares knowledge with coworkers and educates patients and their families to meet health needs. Participates in quality and process improvements to attain clinical excellence. Promotes financial stewardship by assisting with cost reduction without sacrificing quality. Maintains a safe and clean working environment.
