Stroke/Sepsis Coordinator

Job Summary: The Stroke & Sepsis RN Coordinator is responsible for the strategic development, implementation, and ongoing management of protocols, procedures, and performance improvement initiatives related to stroke, sepsis, and community screening care as a Patient Navigator. This role ensures early identification, appropriate treatment, accurate documentation, and improved outcomes for patients with actual or suspected stroke and sepsis across the Emergency Department, inpatient units, and ICU.
Serving as a clinical leader, educator, liaison, and data analyst, the Coordinator plays a key role in aligning care practices with Joint Commission certification standards, Title 22, CMS Core Measures, AHA Get With The Guidelines (GWTG), and hospital goals. The position actively contributes to department operations and communications, and consistently upholds the mission, vision, and values of Pipeline Health. It also promotes excellence in patient communication through the AIDET framework (Acknowledge, Introduce, Duration, Explanation, Thank You).

Essential Functions:
Leads the development, implementation, and continuous improvement of evidence-based stroke, sepsis and community screening for STIs care as a Patient Navigator in partnership with Public Health Institute Bridge program. Along with workflows, protocols, policies, and procedures across Emergency Department and inpatient departments. Coordinates interdepartmental communication and planning to support compliance with Primary Stroke Center (PSC) standards and SEP-1 metrics. Provides education and training to employees, including nurses, physicians, and support personnel, to enhance their understanding of sepsis recognition, early intervention, and appropriate treatment. Implements screening tools and procedures to identify patients at risk for sepsis and facilitate early detection. Collects, analyzes, and reports sepsis-related data and outcomes to identify areas for improvement and to monitor the effectiveness of sepsis protocols. Collaborates with interdisciplinary teams to ensure the seamless integration of sepsis protocols into patient care, including the development of care pathways. Participates in quality improvement initiatives related to sepsis care and contributes to reducing sepsis-related morbidity and mortality. Oversees the ongoing monitoring of sepsis cases, tracks compliance with sepsis protocols, and reports data to relevant authorities and leadership. Promotes community education and awareness about sepsis, its prevention, and early recognition through workshops, seminars, or outreach programs. Ensures compliance with local, state, and national regulations and guidelines related to sepsis care and reporting. Uses clear, concise, professional communication with coworkers, patients, all customers internal and external. Uses AIDET in interactions with patients and family members. Utilizes white boards for patient communication. Collaborates across disciplines to assist in coordination of patient care, including but not limited to patient transfer, discharge, referrals, and spiritual/psychosocial support needs. Acts with a sense of urgency when performing tasks. Basic unit/department maintenance such as keeping files, drawers, cabinets free from unnecessary clutter. Reports on any equipment and or environmental issues for repair. Abides by HIPAA (Health Insurance Portability and Accountability Act) regulations. Utilizes SBAR (Situation, Background, Assessment, Recommendation) when communicating with oncoming shift, physician communication and giving report to ancillary or other nursing services. Speaks up to stop the line and escalates potential safety events if necessary. Completes and attends monthly training assigned. May float to other areas of the hospital as needed. Qualification/Experience:
Minimum three (3) years’ experience as a nurse in an acute care setting. Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, strong organizational skills, ability to follow verbal instructions, and PC (computer) skills. A capacity to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with the team members who are recognized leaders within health care.
