PHP Director of Clinical Operations
has a passion for overseeing the delivery of quality care to seniors and other residents in a variety of healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth. Job Summary : The Personalized Health Partners (PHP) DCO is responsible for overseeing high-quality primary care within a high touch, person-centered care model. The PHP DCO manages clinical operations, staff, and resources, including hiring, training, and performance management. PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS Ensure the delivery of high-quality patient care and compliance with regulations and standards. Provide ongoing performance feedback, coach employees, and coordinate disciplinary actions or terminations with Sr DCO. Identify educational needs and collaborate with educators to develop and implement training. Develop and implement operational strategies for the clinical team. Review clinical opportunities and recommend strategic improvements. Maintain compliant documentation within our EMR system. Develop relationships with key stakeholders within SNFs. Support strategic initiatives related to value-based care. Provide excellent customer service to foster satisfaction and retention. Practice ethically, adhering to professional standards and Board Certification. Follow all applicable state and federal regulations, guidelines, and laws. Additional duties of DCOs Collaborate with facility leadership teams. Be present in assigned facilities at least monthly to ensure adequate NP coverage. This role is field-based (at least 60% in the field, 40% administrative). Provide weekly performance reviews to NPs regarding productivity, visit completion, unsigned notes, canceled visits, and training completion. EDUCATION and QUALIFICATION: Required: Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification from AANP or ANCC. Active, unrestricted state license(s). Ability to obtain licensure in additional states as needed (OH, IN, WV, MD, VA, PA, MO). Active DEA license. US work authorization. 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice. Ability to work flexibly and manage schedules. Passion for healthcare delivery to seniors and low-income populations. Preferred Qualifications: 1+ years clinical management experience as a Nurse or Nurse Practitioner. Geriatrics training or experience. 3+ years clinical experience as a Nurse Practitioner. Experience with coding, ICD-10, and HCC. Experience in results-driven healthcare models. Willingness to pursue professional development and certifications. Experience with value-based care models. Geriatric, assisted living, or skilled nursing experience. Collaborative healthcare experience to achieve positive outcomes. Experience in outpatient primary care settings. JOB SKILLS Strong communication skills (oral and written). Detail-oriented with ability to work independently and in teams. Customer service mindset with residents and teams. Excellent time management and prioritization skills.
