The Care Manager is responsible for conducting facility and community based comprehensive biological, psychological, social, level-of-care, and spiritual assessments; making appropriate referrals to community and treatment supports; and coordinates level of care transition needs. In addition, provides resources and education to both persons served and family members of persons served. Job Duties And Responsibilities:

Assess the individual's unique strengths and needs. Coordinate and facilitate the development of Individual Support Plan (ISP) using a person center planning approach to meet the individuals' needs in the least restrictive manner possible. Validate that service objectives and outcomes are consistent with the individuals' needs and desired outcomes. On-going management of the ISP in cooperation with individual, his/her family, members of ISP team, and providers of service. Periodic review of the ISP, including the update of the ISP at least annually and whenever the individual's needs change. Ensure the distribution of ISP to all team members. Assist with locating services & supports as assessed by individual & his/her family by linking, arranging for or obtaining services specified in the ISP (including needed medical, social, habilitation, education, community services, and other natural support network). Utilization of PUN's to address any unmet needs, assess urgency of need. Periodic review of PUN's including update at least annually or whenever the individual's needs change. Coordinate support planning with providers of service to ensure consistency of services. Facilitate the resolution of any barriers in service delivery, health, and or welfare & documentation of closing the loop. Monitoring the health & welfare of individual Monitoring ISP implementation, and health and welfare through visits with individual, family & providers of service (as determined by frequency stated in ISP & ODP's stated regulation). Monitor individual and his/her family satisfaction with services Evaluate individuals' progress. Participate in SIS assessment & utilization of information gained. Coordinate & participate in PCHC reviews & HRP reviews. Obtain competencies in medical issues & genetic diagnosis Advocate for continuity of services, system flexibility & integration. Working knowledge of current service definitions. Working knowledge of funding streams, discussion of service preference & referral process. Assess the proper utilization of facilities and resources. Assess effective use of individuals' budget & utilization projected & authorized units. Documentation of all support coordination activities in HCSIS (service notes, PUN's, ISP, monitoring tools) On going assessment of individuals' eligibility for waiver funded services. Participate in 24 hours of training per fiscal year Adhering to all ODP's requirements. Physical And Sensory Requirements:

Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability to frequently use fingers/hands for fingering and handling. Must have the ability to use upper extremities for frequent pushing of objects up to 10 pounds and occasional pushing of objects up to 75 lbs. Must have the ability to transport patients weighing up to 250 lbs. via wheelchair, stretcher and/or bed. Must have the ability for frequent stooping and bending. Must have the ability for frequent reaching above shoulder level. Must have the ability for touching as it relates to feeling objects by touching with the skin, particularly that of fingertips. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision. Education:

Bachelor's Degree with major course work in human services, sociology, psychology or other related social sciences plus one year of experience working in the intellectual disabilities field or related experience. Training And Experience:

Demonstrate an understanding of individuals with developmental delays and sensitivity in working with consumers and family. Works with consumer and significant others in a manner which demonstrates respect and concern for the person's welfare.
