Director - System Care Coordination
to join our team at the
Aspirus Wausau Corporate Office ! The Director System Care Coordination is responsible for system-wide Care Coordination functions (Utilization Review, Social Work, Navigation, and Case Management). This includes executing the care coordination strategic plan for the health system and health plan, organizing and directing an integrated cross-continuum care management and transitions of care program. The role monitors (both individual cases and aggregate data), directs actions to meet health system and health plan goals, initiates change management processes, and implements best practices, policies, and procedures. The position oversees all Aspirus entitieshospitals, clinics, post-acute care, and contracted care coordination services through Aspirus Business Health and the Aspirus Health Planand supervises all healthcare providers under their direction. Responsibilities also include recruitment, retention, performance reviews, professional development, and the delivery of care management and transition services. Experience/Qualifications: Knowledge of patient care standards and management practices in healthcare, typically acquired through a bachelors degree in Nursing. Masters Degree required in business, healthcare administration, or nursing. Extensive knowledge of payer mechanisms and clinical utilization management. Minimum of five years in care management or utilization review. Five to seven years of leadership experience in care coordination, case management, or utilization review within an integrated healthcare system and/or health plan. Active Wisconsin or Michigan RN license. Nationally recognized certification preferred (e.g., CPHQ, CCM, CPUM, CPUR, D-ABQAURP). Knowledge of HEDIS, MIPS, Core Measures, NCQA. Proficiency with clinical systems, outcome measures, and patient information systems like Pathways, HEDIS, etc. Understanding hospital information systems and their benefits. Strong data analysis and interpretation skills; data-driven with a passion for medical management. Ability to organize and present data clearly to peers, physicians, and stakeholders.
