Shift Type:
Full-Time Op M-TH 7:00 am-4:30 pm, Fridays off (with the exception of occasional Friday training.) No weekends or major holidays.

Patient Care Coordinator
to join our team and play a key role in supporting patients throughout their healthcare journey.
Job Summary:
The Patient Care Coordinator serves as a vital link between patients, clinical staff, and providers. This role ensures that patients receive the information, support, and scheduling assistance they need, while also helping to streamline clinic operations and enhance the overall patient experience.
Key Responsibilities:
Greet and assist patients in person and over the phone with professionalism and empathy
Schedule and confirm appointments and follow-ups
Collect necessary documentation
Serve as a point of contact for patients regarding their treatment plans, questions, and concernsCoordinate referrals, and communication between departments or specialists
Maintain accurate and up-to-date patient records in the EHR system.Ensure patients are informed and prepared for their visits
Work closely with providers, nurses, and administrative staff to support seamless patient care
Qualifications:
High school diploma or equivalent (Associate's or Bachelor's degree preferred)
1+ years of experience in a medical office or healthcare setting
Strong interpersonal and communication skills
Excellent organizational and time management abilities
Familiarity with EHR systems (e.g., Epic, Athena, etc.) preferred
Ability to multitask in a fast-paced environment with attention to detail
Compassionate, patient-focused attitude
