Provide individualized physical therapy services to residents Develop treatment plans to improve mobility, strength, and independence Collaborate with interdisciplinary team members to ensure holistic care Document resident progress in compliance with state and federal guidelines Educate residents and families on safe mobility and home programs Qualifications
Current California Physical Therapist (PT) license required Strong clinical skills and passion for patient-centered care Excellent communication and teamwork abilities SNF or post-acute experience preferred, but not required (mentorship available)
