Details: Setting: HYBRID (Home Visits & Virtual) Location: San Antonio, TX Schedule: Mon-Fri (8a-5p) Duration: Permanent Duties: Core NP (Homecare): Travel to patients homes Conducting assessments both in patients homes and in virtual environment Counsel and educate patients and families about benefits and programs available.
