Home Health Aide

Location -Trinity Health PACE - New Jersey

Hours - On- Call

Position Purpose

Trinity Health PACE is seeking a compassionate Home Health Aide to support the personal care and health needs of our elderly participants. You'll play a vital role in helping participants maintain independence and dignity in their daily lives.

What You Will Do

Provide personal care assistance including bathing, grooming, toileting, and mobility support

Monitor and report changes in participants' physical or emotional condition

Support participants during recreational activities, meals, and appointments

Ensure safety and infection control standards are met

Collaborate with the interdisciplinary care team and maintain accurate documentation

Assist with transportation and home care services as needed

Minimum Qualifications

High school diploma or equivalent

Active Home Health Aide certification (Certified Nurse Assistant preferred)

CPR certification required

At least 1 year of experience with frail or elderly populations (or willingness to train)

Valid driver's license, car insurance, and reliable transportation required

Strong communication, teamwork, and organizational skills

Pay Range - 17.53-25.42
