JOB DESCRIPTION
Summary
Department:
SNF Therapy
Shift timings : 08:00 - 16:30 Monday to Friday 5 days/8 hours
Per Week : 40 Hours
Occasional Saturdays if willing Would like if they have Home Care Home Base experience but not required. Roles & Responsibilities
Consulting with patients to learn about their physical condition. Assessing and interpreting patient evaluations and test results. Developing treatment plans using a variety of treatment techniques. Creating personalized fitness-oriented health care programs for patients. Administering medically prescribed physical therapy treatments to relieve pain and improve mobility. Advising patients on exercise techniques. Advising patients and their families about in-home treatment options. Providing educational information about injury prevention, ergonomics and ways to promote physical health. Consulting and collaborating with other healthcare professionals. Documenting patient care history. Complying with rules, regulations, and procedures. QUALIFICATIONS
BLS and ACLS National Institutes of Health (NIH) Sound knowledge of anatomy, physiology, pathology, and medical terminology. The ability to stand for extended periods. Strong technical skills. Knowledge of medical terminology Passionate about patient care Requires a minimum of two years of current Physical Therapist experience. Graduate from a fully accredited Physical Therapist program Active unencumbered Physical Therapist Idaho state license One year of Cerner experience Requirements • COVID vaccine with 2 Shots & Booster.

Maintains and updates clinical knowledge and skills. Participates in relevant continuing education programs. Knowledge of and dedication to the highest patient care and provide the utmost service excellence to all patients. Ability to take initiative and to exercise independent judgment, decision-making and problem-solving expertise. A can-do, team oriented attitude is essential, as the ability to work independently and to interact easily with physicians, other managers and staff throughout the department and hospital. Effective interpersonal/communication, organizational, and the flexibility to handle multiple tasks.
