BECOMING OUR HOME HEALTH PATIENT CARE MANAGER: This is a full-time position based in Fullerton, CA, with limited travel to meet client care needs. You may be required to travel up to 10% of the time, including attendance at job-related training or conferences. On-call participation and flexible availability may be required to ensure 24/7 client care coverage. As our Home Health Patient Care Manager, your day will be a dynamic blend of coordination and compassion. You'll oversee scheduling for our field staff, ensuring each client is matched with the right caregiver and that care is delivered seamlessly. You'll follow a structured process for client touchpoints, support the intake and conversion of new clients, and be the key liaison between families, staff, and clinical leadership. From making sure client care surveys hit that perfect 10, to stepping in as a field caregiver when needed, you'll be the thread that holds our care delivery system together. You'll also help maintain accurate billing and documentation, all while driving the high-touch, high-quality standards that BrightStar is known for.

What We Need From You: ● High school diploma ● Licensed Vocational Nurse (LVN) ● Current CPR certification ● 2+ years of experience as a Customer Care Manager and/or managing customer service within a healthcare office setting ● Knowledge of HIPAA and healthcare office regulations ● Proficiency with computers, the internet, and MS Office (Outlook, Word, Excel, and PowerPoint) ● Ability to provide a negative TB skin test or chest x-ray ● Ability to meet physical job demands, including lifting up to 50+ lbs. ● Excellent organization, planning, and project management skills ● Valid driver's license with reliable transportation and proof of insurance
