CC MHS Intensive Outpatient Services Hours:

36.00 per week

Position Summary To provide direct services as a part of a multidisciplinary team the Licensed Clinician (LCSW, LPC, or LMFT) will provide direct care including individual and group therapeutic services to patients, including assessments, treatment plans and discharge planning. Essential Duties & Responsibilities Direct Patient Contact: Participates in direct care to assess needs and coordinate services to appropriate population

Provides initial mental health assessments resulting in measurement based care planning

Assesses using standardized screening tools, measures, etc

Provides brief, solution focused therapy

Provide group psychoeducation/treatment related to specific diagnoses/presenting issues

Provide information and referral services

Participate in care coordination

Maintains daily/weekly scheduled appointments in electronic records systems

Provide therapeutic services to patients and their families

Work in collaboration with multidisciplinary team members including external partners and agencies

Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner

Participate in our high risk program

Demonstrates cultural competency and commitment to patients including LGBTQ+ community

Maintain case load dependent on need

Provide supervision and clinical oversight to master’s level students/interns

Provides mentoring and training to newly hired staff as appropriate

Minimum Qualifications

Master’s degree in social work, counseling, or marriage and family therapy Possession of valid clinical license in Connecticut Ability to diagnose using DSM 5 Must have excellent writing and communication skills, be highly organized Must be able to work as a part of a team

Preferred Qualifications

Previous experience providing direct clinical care is desirable

Additional certifications in specialized clinical care such as DBT, CBT, trauma informed care, etc.

Experience/knowledge of substance use disorders

Experience/competency in working with LGBTQ+
