Position Overview:
Location: San Antonio, Texas
Schedule: Full-time, Monday Friday (5 days/week)
Setting: Skilled nursing facilities (post-acute care)
Employment Type: W2, benefits-eligible
What You'll Do:
Provide primary care to adult patients in post-acute settings
Manage chronic conditions, coordinate care, and support patient goals
Collaborate with interdisciplinary teams and facility staff
Document care using EMR tools, with dedicated training and support
Contribute to quality improvement initiatives and patient safety programs
Ideal Candidate:
Board certified in Internal Medicine or Family Medicine
Active Texas medical license and DEA
Post-acute care experience is a plus, but not required
Strong interpersonal and clinical skills
Interest in leadership opportunities and professional development
