Job Description Job Title: Certified Occupational Therapy Assistant Profession: Rehabilitation Therapy Specialty: COTA Duration: 13 weeks Shift: Monday to Friday, Days Hours per Shift: 32 Experience: Minimum years of experience required License: Valid state license required Certifications: PPD/Test reports, Negative drug screen results, LA Fire Safety Card (Every 4 years)
Description: Please review the credentialing checklist prior to submission. License verification is required. A resume must be provided. Three references are needed. County criminal background check for the last 7 years is required. Federal criminal background check for 7 years or more is necessary. A multi-state criminal search for 7 years or more is needed. A nationwide sex offender search for 7 years or more is required. HHS/OIG search is mandatory. GSA search must be conducted. Social security number trace is required. A 1-step PPD or Quantiferon test or proof of a positive PPD with a chest X-ray is necessary. SAM search results should be negative. Physical examination is required. Position urgency is normal.
