Description Conduct thorough evaluations of patients in both inpatient and outpatient settings while formulating personalized treatment plans Implement targeted physical therapy interventions designed to enhance mobility, strength, balance, and overall function Work closely with a team of nurses, physicians, case managers, and rehab professionals to ensure optimal health outcomes for patients Provide education to patients and their families regarding therapeutic objectives, discharge planning, and at-home exercise regimens Maintain diligent documentation of patient evaluations, treatments, and progress, following all facility and regulatory guidelines Engage in collaborative care team rounds and interdisciplinary conferences to guarantee seamless care delivery Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
Current state licensure as a Licensed Physical Therapist CPR certification is preferred Specialized certifications such as neurological or geriatric therapy are beneficial but not mandatory Skills
Prior experience in a clinical setting is strongly preferred Proven competency in treating patients with complex medical and mobility challenges Excellent communication and teamwork capabilities Familiarity and comfort with digital charting and clinical documentation systems Benefits
