About the Role:
We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to deliver holistic, patient-centered medical care in home and community-based settings, such as adult foster care homes. You will collaborate with a multidisciplinary team-including nurses, social workers, behavioral health counselors, dieticians, and the medical director-to improve patient health outcomes, engagement, safety, and quality of life.
Responsibilities:
Build positive rapport and engagement with patients and coworkers while ensuring excellent customer service.
Conduct comprehensive clinical assessments, including medical history, medication reconciliation, vital signs, and physical exams.
Maintain accurate and timely clinical documentation.
Review and interpret medical, surgical, and social history; perform appropriate screenings.
Diagnose conditions, develop and update individualized Plans of Care, and involve patients and support systems in decision-making.
Qualifications:
Graduate degree in Nursing (Nurse Practitioner) or Physician Assistant Studies.
Valid Michigan driver's license.
Current Michigan NP or PA license.
Current national certification (AANP/ANCC for NPs; NCCPA for PAs) in Adult, Geriatrics, or Adult Gerontology Primary Care.
Minimum 2 years of experience in Family or Primary Care.
