MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency's clinics. This includes diagnosis, treatment, consultation and follow-up care. This individual attends to the patient's need with the utmost patient care, compassion and understanding. This work is carried out in support of the mission. DUTIES AND RESPONSIBILITIES:
- Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program.
- Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
- Records accurate medical histories, physical findings and formulates plan and prognosis based on patient's condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation.
- Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan.
- Submits health care plan and goals of individual patients for periodic review and evaluation by physician.
- Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures.
- Refers patients to physician or to specialized health resources for treatment.
- Makes patients aware of importance of a healthy lifestyle.
- Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments.
- Analyzes new data gained from conferences, training workshops, professional literature and on the job training, where they will assimilate it into their clinical work.
- Keeps abreast of changes in field.
- Performs other duties as requested.
- Master's degree from an accredited advanced Nurse Practitioner program
- At least Two (2) years' experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs
- Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs
- Ability to demonstrate sensitivity to patients with a variety of psycho social issues
- NYS RN and NP (Adult and Family specialty areas) licensures.
- Valid DEA registration.
- eMedNY Provider.
- Valid NPI.
- BLS Certification.
- National Board Certification within 12 months of hire.
- ACLS certification.
- Familiarity with OASAS, OMH, and DOH regulations.
- Healthcare
