We are seeking acompassionate and skilled Nurse Practitioner or Physician Assistant to join ourteam in Hominy, Oklahoma. This full-time position offers the opportunity toprovide comprehensive primary care services in a community health setting. Youwill work collaboratively with a multidisciplinary team to promote health,prevent illness, and deliver high-quality care to patients across the lifespan.New graduates are welcome to apply.


Key Responsibilities:

  • Provide comprehensive healthassessments and primary care services, including screening, diagnosis,treatment, and health education, within the NP/PA scope of practice.
  • Conduct systematichistory-taking and physical examinations.
  • Compile and maintainaccurate patient medical records and health histories.
  • Order, perform, andinterpret diagnostic and laboratory tests; identify deviations from normalresults and initiate appropriate follow-up.
  • Develop and implementindividualized treatment plans based on clinical findings.
  • Provide patient counseling,education, and follow-up for acute and chronic health conditions.
  • Collaborate with physicians,nurses, and other healthcare professionals in the delivery of coordinated care.

Requirements:

  • Graduation from anaccredited Nurse Practitioner or Physician Assistant program.
  • Current licensure as a NursePractitioner or Physician Assistant in the State of Oklahoma.
  • National board certificationas a Nurse Practitioner or Physician Assistant
  • Current DEA and Oklahomaprescriptive authority.
  • Current BLS/CPRcertification
  • Excellent clinical,diagnostic, and interpersonal skills.
  • Strong organizational skillsand the ability to work both independently and as part of a team.
  • Experience in familymedicine, internal medicine, or community health preferred.
  • New graduates with strongclinical rotations in primary care are welcome to apply.
