Family Practice/Primary Care Nurse Practitioner

We are seeking afull-time Family Medicine Nurse Practitioner to join our expanding primary careteam in Richmond, VA. In this role, you will provide evidence-based care to adiverse patient population with an emphasis on prevention, chronic diseasemanagement, and whole-person health. You will practice in a collaborativeenvironment alongside a dedicated Whole Health Coach and a supportive clinicalteam, with a model designed to prioritize quality of care over volume.

Position Highlights:

• Patient Panel: 10-15patients per day - time to focus on what matters
• Schedule: Monday-Friday,with occasional regional after-hours coverage rotation
• Model of Care: Medical Homemodel with onsite lab and pharmacy for convenience

Compensation & Benefits :

• Compensation: $140,000+annual salary with annual performance reviews

Requirements:

• Board-certified NP with abackground in family medicine, internal medicine, or primary care
• Active and unrestrictedWashington license and DEA
• 3-5 years of primary careexperience
• 2+ years in case managementor population health preferred
• Familiarity with EMR systems(NextGen preferred) and Medicare Advantage/HCC documentation
