Serves as a patient advocate at all times. Supervises the nursing care provided to each patient by other caregivers. Develops the plan for nursing care as a result of coordination with the patient, family, and staff of other disciplines as necessary. Applies critical thinking in order to promote positive clinical outcomes. Ensures patient education and participates in discharge planning. Maintains the standard of nursing care and implements the policies and procedures of the department. Assesses the patient on admission within the required time frame. Makes referrals to other disciplines based on the assessment, such as rehabilitation and nutritional services. Develops a nursing care plan for assigned patients on admission and updates the plan of care as needed. Ensures the plan is discussed and developed as a result of the coordination of patient, family, and interdisciplinary team. Able to release, remove, and reapply restraints to patients. Ensures patient is in proper position and alignment. Documents patient admission assessment and reassessments, patient care plans, and other pertinent information completely in the electronic medical record. Demonstrates correct use of biomedical equipment such as defibrillators, suction machines, IV and feeding pumps, PCA pumps, and blood glucose monitoring machines. Demonstrates safe and effective delivery of patient care, completing team/patient assignments on time, making effective use of available time, and showing initiative and teamwork. Obtains consents for procedures and blood transfusions, ensuring patients have necessary information to make informed decisions. Receives physician orders, ensures transcription is accurate, and order entry is correct. Follows through to ensure orders are carried out. Recognizes and responds appropriately to changes in patients' conditions, such as cardiac rhythm, respiratory distress, GI bleed, pulmonary embolism, diabetic ketoacidosis, adverse drug reactions, and blood transfusion reactions. Institutes emergency actions, notifies physicians, and obtains orders as needed. Completes and documents patient admission assessments and reassessments as required according to policy. Reports pertinent observations and reactions regarding patients to the appropriate persons and records these observations accurately. Administers medications with knowledge of action, side effects, correct dosages, and route according to policies and procedures. Demonstrates the ability to recognize and institute arrhythmia protocols for various cardiac rhythms. Directs, supervises, and evaluates nursing care provided to patients. Administers medications utilizing the seven rights of medication administration: right patient, route, time, medication, dose, reason, and documentation. Assists physicians with special tests and procedures as needed. Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner. Participates in orientation and performance evaluations of nursing staff. Assists team members in providing care to patients or administers direct care as needed. Coordinates nursing care of patients scheduled for therapy or procedures by other departments. Assists with or institutes emergency measures for sudden adverse developments in patients. Participates in maintaining a clean and safe environment. Provides assessment utilizing current ACLS certification and protocols, rendering lifesaving first aid and making appropriate referrals. Demonstrates ability to manage various types of tubes and devices. Demonstrates correct setup and performance monitoring of hemodynamic systems. Demonstrates accurate knowledge of critical lab values and their normal ranges, reporting and documenting appropriately. Communicates pertinent information to RNs, physicians, and patients as necessary. Adheres to dress code and maintains a professional appearance. Completes annual health, safety, and education requirements. Maintains professional growth and development while ensuring confidentiality of all patient information. Demonstrates knowledge of age-appropriate care for the patient population served. Attends mandatory in-services and staff meetings. Represents the organization professionally and complies with organizational policies regarding ethical practices. Maintains current licensure and certification as applicable. Demonstrates excellent oral and written communication and interpersonal skills. Shows knowledge of medications and correct administration based on age of patients and clinical conditions. Organizes tasks, develops action plans, sets priorities, and functions under stressful situations. Maintains good working relationships with co-workers. Reads, writes, and speaks fluent English.
