Seniors At Home offers a wide range of services to help older adults live safe, healthy, and independent lives, including home care, personal assistant services, companionship, and specialized dementia and palliative care. We provide the Bay Area's leading continuum of care for aging adults.
Position Summary Under the supervision of the Director of Home Care, the Home Care Client Services Manager plays a central role in ensuring high quality, compliant, and compassionate delivery of non-medical home care services within the Seniors at Home (SAH) program. This role is responsible for the onboarding and assessment of private pay clients, development of individualized care plans, caregiver supervision, compliance with the California Department of Social Services (CDSS) Home Care Services Bureau (HCSB) requirements, and overall client relationship management.
Compensation and Benefits
Salary Range: $70,000-$80,000 (depending on experience)
Employment Type: full-time position with benefits
Employer 403(b) retirement match plus additional employer contribution (subject to eligibility)

16 holidays (10 federal and up to 6 Jewish holidays), annually

Essential Duties and Responsibilities
Conduct initial in-home and pre-discharge assessments to determine client needs
Serve as the primary contact for assigned clients, providing ongoing communication and support
Oversee caregivers assigned to caseload, ensuring full compliance with CDSS Home Care Aide Registry requirements
Conduct in-home supervisory visits, performance evaluations, and documentation of staff performance observations
Maintain timely and accurate documentation in the agency's client management system
Participate in the on-call rotation for urgent client or caregiver needs outside of business hours
Travel as needed throughout service area for client visits, caregiver supervision, and community outreach
Build and maintain referral relationships with hospitals, facilities, and community organizations
Job Qualifications
A Bachelor's degree in Social Work, Nursing, Gerontology, Human Services, Public Health, or related field is required
Minimum of 3 years' experience in home care, aging services, or related healthcare field
Familiarity with California CDSS Home Care Organization (HCO) regulations required
Prior experience conducting assessments and supervising caregivers strongly preferred
Bilingual fluency in Russian and/or Tagalog is desirable
Intermediate proficiency in Microsoft Office Suite; Experience with Electronic Health Records (EHR) and home care scheduling or compliance platforms is preferred
Exceptional written and verbal communication skills. Able to clearly and professionally convey information, navigate sensitive conversations, and represent the agency with stakeholders
High emotional intelligence, cultural competence, and previous experience in supervising caregivers or front-line staff, is preferred
Valid California Driver's License, access to reliable transportation, and proof of automobile insurance coverage is required
