Sign-On Bonus:

$15,000.00 (Conditions Apply)

The posted compensation range of $60.00 - $72.42 per hour is an estimate based on current circumstances. Pathways Home Health & Hospice may pay more or less than this range, as permitted by law.

Position Summary: The goal of physical therapy is to restore function, prevent or minimize disability following disease or injury, and help clients reach their maximum performance within their capabilities. The Physical Therapist evaluates and treats patients using medically prescribed programs and may supervise Physical Therapy Assistants according to agency and regulatory standards.

Areas of Responsibility:

Conduct initial admission/evaluation visits, notify Clinical Team Manager of OASIS scores, and inform staff of admissions and follow-up plans.

Develop, re-evaluate, and revise the plan of care in collaboration with the Clinical Team Manager; instruct PTAs on plan revisions.

Document observations, clinical findings, interventions, goals, and discharge plans regularly.

Assess the need for other team members' services (RN, OT, ST, MSW, HHA).

Provide and document skilled care/interventions during visits, including patient condition observation, medical procedures, and teaching.

Obtain and document physician orders.

Perform supervision of Home Health Aides as required by regulation.

Coordinate care and communicate significant patient changes with the care team, physicians, and other members.

Adhere to standards for documentation, point-of-service technology, and communication devices.

Submit weekly schedules, collaborate on scheduling, and handle re-certifications, transfers, and discharges timely.

Supervise Physical Therapy Assistants.

Qualifications:

Graduate from an accredited physical therapy program approved by the American Medical Association and the American Physical Therapy Association.

California license as a Physical Therapist.

At least 2 years of recent experience as a Physical Therapist; interest in gerontology and home care preferred.

Current CPR certification.

Valid California driver’s license and automobile insurance.
