We are currently looking for a Full Time Certified Home Health Aide to join our Home Health Clinical team located at the Chickasaw Nation Medical Center working 8:30AM-5:00PM - Mo Tu We Th Fr.
Summary:
The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care. The home health aide will not function in any manner viewed as the practice of nursing according to the Oklahoma Nurse Practice Act. Specifically, the home health aide will not administer the medications, take physician's orders, or perform procedures requiring the training, knowledge, and skill of a nurse, such as sterile techniques.
Responsibilities:
* Provides hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linens, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, and keeping the patient's living area clean and orderly, as possible
* Plans and prepares nutritious meals
* Assists in feeding the patient, if necessary
* Takes and records oral, rectal, or axillary temperatures, pulse, respirations, blood pressure, and weight when ordered (with appropriate completed/demonstrated skills competency)
* Assists in ambulation and exercise according to the plan of care
* Performs range of motion and other simple procedures as an extension of therapy services as ordered (with appropriate completed/demonstrated skills competency)
* Performs simple procedures as an extension of nursing services as ordered (with appropriate completed/documented skills competency)
* Assists the patient in the self-administration of medication
* Assists with patient's laundry, as appropriate
* Meets safety needs of patients and use equipment safely and properly (foot stools, side rails, etc.)
* Reports on patient's condition and significant changes to the assigned case manager
* Participates as a member of the interdisciplinary team
* Provides first class customer service - courteous and helpful to all internal and external customer types

* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types

- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
- Adheres to Chickasaw Nation dress code standards
* Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.
* Performs all other duties as assigned
