Specialty Patient Care Coordinator-Sorter Job Type:

POSITION SUMMARY The Specialty Patient Care Coordinator-Sorter will be responsible for managing incoming referrals by creating charts, uploading documents, contacting patients, and referring providers, and ensuring referrals are routed to the appropriate task box for processing. The ideal candidate will have a background din healthcare and a combination of skills in customer service, referrals, communication, behavioral health, coordinated care and insurance.
Essential Functions Statement(s)
Referral Management Receive and process incoming referrals from providers. Create patient charts in the electronic health record (EHR). Upload referral documents into patient records. Verify accuracy and completeness of referral information. Ensure referrals are routed to the correct task box for further processing. Communication and Coordination
Contact patients to review referral information and explain next steps. Communicate with referring providers to obtain missing information or clarify referral details. Provide excellent customer service to patients and providers throughout the referral process. Documentation and Tracking
Maintain accurate, up-to-date records of referrals received and processed. Track referral progress to ensure timely follow-through. Identify and resolve issues that may delay referral completion. Team Support
Collaborate with team members to ensure efficient workflow. Assist with additional duties as requested by supervisors. Demonstrate reliability and a willingness to take on added responsibilities as needed. SKILLS & ABILITIES Education : High School Diploma or GED Experience : Experience in customer service, healthcare, or related field required. \
